What is Protopic?

Protopic is part of a group of medicines known as topical calcineurin inhibitors (TCI), which are considered 'immunomodulating agents'. The active ingredient in Protopic is tacrolimus monohydrate (tacrolimus). This ointment is one of two TCIs available, with the other being pimecrolimus cream, marketed under the brand name Elidel.

Vitiligo is an autoimmune condition where your immune system attacks melanocytes, the cells in your skin that produce melanin. Melanin is what gives colour to your skin, hair, and eyes. When there's not enough melanin, pale patches appear on the skin of those with vitiligo. These lighter patches can form anywhere on the body, but they often appear on the face, hands, neck, and in the folds of your skin.

How does Protopic work?

TCIs, such as Protopic, help treat vitiligo by calming your immune system. This prevents it from attacking melanocytes, allowing melanin to be produced. As a result, the colour of the affected areas of your skin can improve.

It may take several months for your skin pigment to change as much as you'd like, but using tacrolimus twice daily has been shown to help treat vitiligo.

What doses of Protopic are there?

Protopic ointment comes in two strengths: 0.03% and 0.1%. Your clinician will recommend the strength they believe is most suitable for your condition.

For eczema, you'll usually apply the ointment twice daily—once in the morning and once in the evening. This could be the routine you're recommended to follow if you're trying to change your skin pigment, but it can vary from person to person.

A study from 2009 found that applying tacrolimus twice a day was an effective way to treat facial vitiligo.

How long does Protopic take to work?

Since Protopic is used off-label for treating vitiligo, there isn't much research available on how long it takes for the ointment to impact skin pigmentation. The time it takes can differ from person to person, as each case of vitiligo is unique.

Restoring skin colour can take quite some time, so it may be a while before you notice any changes. Skin pigment changes are usually checked after 6 months, so you might be advised to continue using Protopic for this period before you and your clinician assess if the ointment is effective.

What happens if I make a mistake while using Protopic?

Protopic is more likely to work well if you follow the dosage schedule your clinician has advised. If you forget to apply the ointment at your usual time, just do so as soon as you remember. There's no need to use extra ointment to make up for a missed dose, as this could raise the risk of side effects.

If you accidentally swallow some ointment, seek immediate assistance at your nearest hospital.

How is Protopic different from other vitiligo treatments?

The most common way to treat vitiligo is by using a topical corticosteroid. However, using corticosteroids for an extended period can lead to side effects such as skin thinning and acne.

For this reason, managing vitiligo with these treatments usually requires your GP's supervision.

If you have non-segmental vitiligo, which is the most common type and affects both sides of your body, topical corticosteroids are often prescribed. This is typically the case if the condition covers less than 10% of your body.

Protopic is a topical calcineurin inhibitor (TCI), similar to pimecrolimus. It works by preventing your immune system from attacking the skin cells that produce melanin, known as melanocytes. This protection allows melanin to be produced, which helps to restore the skin's colour.

Which dose of Protopic works best for vitiligo?

The amount of Protopic you’ll be given will depend on a few things, like the size of the skin area you need to treat and the skin pigment you want to achieve with the ointment.

A clinician will recommend the dose they believe you should begin with. If you're given the lower dose of 0.03% ointment and don't notice the desired results, you can switch to the 0.1% version, provided our medical experts consider it safe.

Will I get any side effects while using Protopic?

We can't guarantee whether you'll experience any side effects when using Protopic. Every medication can cause side effects, but since everyone reacts differently, you might not have any at all.

The patient information leaflet that comes with your medication includes a list of all the potential side effects you might experience. More than 1 in 10 people using the ointment report increased skin sensitivity, tingling, and redness.

A study examining the use of Protopic for treating atopic dermatitis found it to be safe and effective over a six-month period. Both children and adults in the trial tolerated it well, and any side effects, such as a burning sensation after applying it, were short-lived. This suggests that Protopic is generally a safe treatment option.

It's probably best to avoid alcohol while using the ointment, as it can make your face and skin feel hot and flushed.

Phenomenal NHa toothpaste

I am fluoride free. It’s toxic AND did you know it oxidizes the tooth from the inside out and actually MAKES your TEETH YELLOW!? Crest even admitted it. Fluorides risks outweigh the teeth benefits but LUCKILY there’s a more superior solution: nano hydroxyapatite for our teeth remineralization needs. It’s safe and proven to be more effective as a remineralizing agent, unlike fluoride which cannot remineralize on its own - it requires the minerals in your saliva to work, so if your saliva isn’t at the optimal levels, it’s useless to teeth. But I have been on a teeth whitening journey - got my teeth whitened at my dentist but I ate a tangerine and they got stained again. (Stupid me. Bye $325!) While I’ve restored the whiteness back with various at home products, it’s been at the expense of my enamel and has caused a lot of sensitivity. Sensodyne was given to me by my dentist but when I saw it visibly yellow my teeth after the dentist’s first bleaching, I went down a rabbit hole to get to the bottom of it as to why. Additionally, my enamel has been getting more and and more clear, and I needed this to be addressed. NHa has been proven to restore enamel which treats tooth sensitivity instead of just numbing the nerves with potassium nitrate like Sensodyne does which treats the symptom and not the cause (tooth demineralization, sigh… such a western medicine move). When I bought this for the first time I was immediately pleased - there’s NO scary flavor (if you’ve ever tried *natural* toothpastes, especially ones of India, You KNOW!). There’s no cardamom or other cinnamon or out of the ordinary flavors here. The consistency is that of normal toothpaste. Great foaming action. Has helped my sensitivity in 2 applications, I’m 100% without sensitivity but even better, my teeth my are less clear. This truly restores enamel. But, here’s my gripes. #1 - I have no idea how much hydroxyapatite is in this. I know it works, but as a consumer, I deserve to know what % of the star ingredient is used in a product that I’m specifically buying it for. #2 - the toothpaste color is… lightish yellow. Think a healthy “mucus yellow” - I’m so sorry for the that description, but when I spit up bits of toothpaste in my sink, it looks like boogers. That’s an issue when I’m busy whitening my teeth at home & I’m avoiding eating foods that stain teeth for at least 2 hours after so that my saliva can naturally seal my dental pores that are being blasted open with peroxide. If I brush my teeth after whitening I’m extremely paranoid that I’m pushing YELLOW toothpaste into my porous teeth, and I don’t need to keep driving myself crazy thinking that the NHa is potentially sealing that color in. I also want to be able to put my NHa toothpaste in clean whitening trays to give myself a mineralizing treatment without worrying about it staining my teeth from prolonged/concentrated contact. Mind you - this is very paranoid of me to think, but I’ve spent a lot fixing my teeth bleaching then restaining fiasco, so I’m not taking any chances. While I don’t think it’s yellowing my teeth at all, because of the color, I’ll never think it’s whitening them. But don’t let this deter you from purchasing. It helps sensitivity after one use, has shown to make my enamel less clear, and has a normal toothpaste consistency and foams up great. I recommend it.
